This field set the staff member you would like to add sickness for.
This field sets the date the sickness bookings should start.
This field sets the date the sickness bookings should end.
Sets the if the sickness booking should include the morning for the selected date.
Sets the if the sickness booking should include the afternoon for the selected date.
